Hee. The common consensus is the guy in the vision was the Alpha. I sill don't know who the bald guy is, or if he's even significant. But he seemed not quite belong to the nest, therefore out of place to me. And you are right, I thought about the guy who attacked Sam and Samuel outside too, but although he's bald, he looked like a thug, whereas the bald guy Dean saw looked more creepy.
The vision about the split cell, twin copy popping out of nowhere probably means multiply itself. So with the Alpha pointing outside, the message probably is "go out there and multiply yourself". Although I hope there's additional information in that vision because otherwise it would be rather redundent if the message is originated from the Alpha - surely the vamps in that nest heard that before.
Most likely you are right in the timing being a coincidence. Although I still wonder if Sam has came back with his psychic ability, probably even stronger than before. I can see that as a secret he wants to keep away from Dean. I'm so very looking forward to the next episode. Hopefully I learn more about what's going on.
