I do think the guy in the vision was the alpha, yes. I just went back to watch that little piece again, and I just realized that one of the twins wasn't there at first, she just sorta appeared, kinda like a ghost, then they both bared their fangs. I don't know what that means. I don't know if she was a clone or what. It's all VERY interesting, and I think it's one of the best things the show has done so far. I hadn't noticed the guy on the balcony so I kept an eye out on him. The vamp that attacks Sam and Grandpa is bald too - but it looks like he has tattoos , and I'm not sure if the first guy had them or not, but I don't think so, so, not sure about that. But I'm more convinced that the alpha was the one in the vision.
Nice catch with the psychic ability. I was really just wondering why it had happened right there and then - but as for Sam being the one who had done it, I don't think so. Maybe it was just a huge coincidence - although, what Dean saw, I really don't think it was a new message to send out so, yeah. Maybe it was more like a connection to the "Alpha" - as in, he saw into the "Alpha"'s memories or something.
I still think of it as an Alpha for each species of monster.
